  • All COPD patient information and appropriate ICD-9 diagnosis codes: 491.0 – 492.8 & 496
  • Patient's primary and secondary insurance
Advise COPD patients to bring primary and supplemental insurance information, along with BROVANA prescription, to pharmacy. Send appropriate information to home health care pharmacy.
